The Summit Club helps many groups and non-profit organizations in the Flower Mound Area.
If you would like us to consider your request for funds or services, please complete and submit the form below.

Please Read!

We highly suggest you follow these recommendations when completing the Service/ Grant Request Form.
1) Use a laptop or desktop computer rather than your phone.
2) Complete the form without leaving the page or closing your computer.
3) For the  a)What are you requesting  and  b)How does request support the mission statement fields
please prepare your response in a text program and paste it into the field.

4) If you receive an error message that says to contact the administrator, please do the following.
a) Copy and save your form information to a text file.
b) Clear your browser cache.
c) Shut down and restart your browser.
d) Try to submit the grant request again.
e) If you still have a problem, please send a note to  vp-is-services@summitclub.org.
